If you are NOT a client and requesting services, you must complete a new client application and submit for approval at least two weeks prior to the needed transportation. Please do not fill out the contact form below, see the Become a Client tab for an application and information.
The Wheels for Wellness office is located at 301 N. Cameron Street, Winchester in the
Shockey Building on the beautiful Our Health campus.
